Your personalized service begins with an Initial In-Home Assessment which is conducted free of charge, by a friendly, experienced Supervisor, called a Client Care Manager.
The purpose of this assessment is to:
- Understand you as a person – your preferences, needs and wishes.
- Understand your medical history, your current needs, and your care goals (what you hope to achieve from our service).
- Give you a chance to ask any questions you may have about our service.
- Clearly discuss your options, including available subsidies or other products and services you may require.
- Develop a care plan and schedule that suits your needs, preferences and budget.
- Meet your family and any other support that may be involved in your care.
- Conduct a Home Safety Inspection on your home and make recommendations for improved safety measures.
After this assessment is complete:
- we will find suitable caregivers that match your health care needs and your personal preferences
- Your personalized schedule is developed and then further discussed with you to ensure complete satisfaction.
- On your first day of service, your Client Care Manager will visit your home with the Caregiver for a First Shift Visit to further ensure your satisfaction.
